Registration 
can be completed online (you’ll need to call 740-833-1100 for a PIN to access the online portal) or at the District’s administration building between 7:30 a.m. and 4 p.m. during the week.

To properly complete the registration the following will be needed:
- child’s birth certificate
- immunization records
- proof of residency
- social security number
- proof of physical examination
- court custody papers (if applicable)
The Administration building is located at 248 North Sandusky Street in historic Northwest Delaware, Ohio.
